The ideal LPN candidate will:

  • Have graduated from an approved school of nursing
  • Be licensed and currently registered to practice as a License Practical Nurse in the State of New York

General Overview of our Licensed Practical Nurse Role:

The License Practical Nurse (LPN) functions under the direction and the supervision of the RN/charge nurse and the Nursing Supervisor. The LPN is accountable to practice nursing as per the Nurse Practice Act, the State Board of Nursing, the ANA's Geriatric Nursing Standards and according to facility policies and procedures.

The nurse is the team leader on the unit coordinating the resident's care in collaboration with the CNAs, physician, and other disciplines through assessment (under the guidance of the RN/supervisor) and involvement with the resident and family. The nurse leads and communicates with the team in the planning and implementation of care and thorough documentation of the same in the EMR. He/she is proficient, or working towards proficiency, in wound care, ostomy care, foley catheter insertion and care, medication administration through all routes including G-tube and IVs, trach care and suctioning. Through keen observation of any actual and potential changes in condition, the nurse together with the other members of the team will respond and react timely and according to protocol to prevent re-hospitalizations and potential emergency situations. In the face of an emergency situation, the nurse responds efficiently and calmly to initiate the rapid response or emergency response team as appropriate.

            The nurse's performance will be evaluated in relation to the ability to demonstrate procedures or behaviors with proficiency and excellence.  The underlying principle of clinical excellence is the careful balance of optimizing safety, quality of life and clinical best practices in a patient specific way.  Therefore, nurses are evaluated in relation to these measures as observed during the orientation period and throughout the year by their manager.
