Fee-For Service Pediatric Physical Therapist

Location: Brooklyn and/or Queens, New York

As the Fee-For-Service Pediatric Physical Therapist, you will be responsible for evaluating the client's level of physical functioning in the home and for working with client, family, and/or others for the planning, provision, and coordination of physical therapy services as per MD prescription. The Pediatric Therapist also works with other professional staff to establish rehabilitation and maintenance goals and activities.

Responsibilities include, but may not be limited to:

  • Developing a plan of treatment which encompasses treatment modalities, frequency of visits, expected outcomes and addresses short- and long-term goals.
    • Assesses patients' needs accurately and develops a therapeutic plan of care to promote patients' maximum level of functioning.
    • Evaluates patients' functional status including ROM, muscle strength, balance coordination, and transferability as well as any functional limitations.
    • Evaluates home appropriately for safety and recommends equipment in accordance with patients' special needs.
    • Considers the patients' psychological response to the effects of illness and subsequent physical changes.
    • Documents frequency and duration on evaluation.
  • Implements the plan of treatment under physician direction and in collaboration with agency professional personnel.
    • Applies physical therapy procedures to achieve identified goals.
    • Instructs and monitors patients, family, and health team members in use of appropriate physical therapy procedures including exercise programs, guarding, and safety.
    • Teaches use of adaptive equipment to client family and other appropriate health professionals.
    • Counsels patient/family to aide in interaction and adjustment to the patients' functional limitations.
  • Observes records and reports to the physician and program personnel the patients' reaction and treatment and any changes.
    • Provides complete verbal report within 24-48 hours of assessment visit.
    • Assists in developing alternate plan options for medically and/or financially ineligible referrals.
    • Maintains communication with other persons providing care and provides information to these persons to further aid in the rehabilitative process.
    • Modifies the plan of treatment as needed and is able to reflect these changes within the written plan of care and progress notes and verbalize the modifications to the CHHA RN.
    • Re-evaluates at the intervals mandated by Medicare guidelines to ensure compliance, detect progress attained, and modify plan as necessary.
    • Discusses re-evaluation with nurse coordinator and submits 13th, 19th visit assessments including patients present status, updated goals, and treatment plans.

Required Qualifications

  • License and current registration to practice as a Physical Therapist in New York state.
  • Graduate from a physical therapy curriculum approved by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association.
  • A minimum of 1 year experience in a hospital or long-term care facility as a Physical Therapist
  • Experience working with a pediatric population is required. A combination of experience working with pediatric, adult, and geriatric populations is a plus.
  • Demonstrated knowledge of Medicare/Medicaid, regulatory, and credentialing criteria and standards.

Physical Demands:

Must be able to make home visits in potentially unsafe and unsanitary home environments and be comfortable with travel on public transportation in potentially unsafe areas of community. Standing, sitting, walking long distances, bending, reaching, stretching, and climbing stairs may be required. Lifting up to 50 pounds unassisted and up to 325 pounds utilizing an assistive device in the home environment may also be required.

Health Requirements:

Adheres to the health requirements of New York State, including employment health assessment and annual health assessment.

Safety Equipment:

Flexible goggles with side shields, anti-reflex respirator, protective gowns, aprons, and gloves as needed.
