Cobble Hill Health Center is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 40 years. The Nurse Manager will play an integral role in helping us to achieve our vision: to consistently exceed expectations and set the standard for excellence in residential and community care.

The Nurse Manager must be an exceptional leader with a track record of creating high performing teams and excellent team culture. They will need to be collaborative, team-oriented, self-motivated and driven, with a strong sense of ownership for their units.

Schedule: Rotating shifts (Weekdays 6am-2pm, 8am-4pm, 10am-6pm and every third or fourth Saturday 7am-3pm)

The ideal Nurse Manager will:

Clinical: Providing excellent care through preventive medicine and the ability to detect a potential decline prior to avoid crisis. This is done through day to day involvement, meaningful and purposeful rounding, and impromptu huddles as necessary utilizing clinical judgment and critical thinking, while engaging all nurses, CNAs, and other disciplines in the process.

  • Meaningful, purposeful walking rounds
  • Arrive half hour before tour ends (vary your times throughout the week)
  • Walk with nurse from prior shift and current shift for meaningful handoff and to prioritize the oncoming shift
  • Assess resident's condition, environment, safety and nursing care
  • Impromptu huddles
  • Clinical Competencies
  • IV insertion and maintenance
  • Reinserts NG and GT as needed
  • Trach and suctioning care
  • Wound care assessment, staging, and treatment
  • Responds to emergencies with ease, clinically sound judgment and initiative
  • Examines and administers emergency treatment
  • Incident/Accident report completed per policy

Managerial: Clearly defined expectations (with proper training) and accountability will drive staff to perform at their highest capacity with satisfaction.

  • Supervises, evaluates, guides, instruct and foster the growth and development of unit personnel the multidisciplinary team on all shifts
  • Adjust schedule accordingly to support all three shifts
  • Prepare assignments of nursing personnel
  • Delegate responsibilities with clear expectations, and follow up on its carry through
  • Conducts regularly scheduled meetings with unit staff
  • Audits continuously evaluates the quality of nursing/unit practices. Results inform areas needed for improvement and implementation of new ideas and approaches. As we learn from these audits, they become training tools for us and our staff. Involve unit staff where appropriate.
  • Corrective action plan to be put in place when an avoidable decline occurs or when resident transferred out to hospital what should we have done to avoid getting to the critical stage?
  • Written corrective action plan around every complaint. This can inform our approach to residents and families, what our customers are looking for in us....
  • Supportive of all facility initiatives and meaningfully champion and actively implement the initiatives on the units

Staffing: Creating a team environment of responsibility, passion, and focus where employees from all shifts feel a united front to care for our residents. It is the hope that with a strong team that feels responsible for one another, there will be no tension between shifts, and fewer call outs as to not leave their colleagues short staffed. A positive and nurturing environment is key to retaining employees.

The ideal candidate should have:

  • A baccalaureate degree in Nursing from an accredited nursing program
  • Be licensed in the state of New York
  • Be certified in BLS and ACLS
  • Must be fully vaccinated for COVID19
